Acceptable Forms of ID
At the time of notarial services, valid identification must be presented. The ID must be current or issued within the last 5 years and must include a serial or ID number. Accepted forms of ID include:

• State-issued driver’s license (FL Department of Highway and Motor Vehicles)

• State-issued identification card*

• U.S. passport (U.S. Department of State)

• U.S. military ID

• State, county, or local government IDs

• Permanent resident card ("green card") issued by U.S. Citizenship and Immigration Services

• Foreign passport (stamped by U.S. Department of Justice, Immigration and Naturalization Service)

• Driver’s license issued by a U.S. Territory, another state, Mexico, or Canada

• Inmate identification cards

Requesting Notarial Services
To ensure a smooth process, please:

1. Identify the type of notarial service you need (e.g., Acknowledgement, Jurat, Oath/Affirmation, Certified Copy, Witness/Attest Signatures).

2. Ensure your identification is valid and matches the name on the document(s) to be notarized.

3. All parties signing must be present with valid ID.

4. Understand the content of the document you are signing.

5. Make sure all applicable blanks in the document are filled out.

6. For Jurats and Affidavits, leave the signature line blank as the Notary must witness the signing.
